29 CFR 1926.1204(h): The employer did not designate each person who was to have an active role in entry operations and did not identify the duties of each such employee, and did not provide each employee with training required by 1926.1207. On or about July 8, 2024, at job site located at 9220 Old Santa Fe Road, in Kansas City, MO, the employer failed to designate roles per person who assisted in or entered the permit space. The company had a 2-person laborer crew plus site foreman who were aware of or assisted in the entry of an active sewer system where the entrant was exposed to atmospheric hazards.
