29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) On or about January 05, 2023 and at times prior to - Building 4, employees were exposed to fall hazards of up to approximately 30 feet when employees accessed scaffolds with deficiencies such as, but not limited to, levels not fully planked, outrigger/sidearm used for other than personnel, missing/incorrect guardrails, missing toeboards, use of damaged scaffold materials, and inadequate safe access to scaffold levels.
