29 CFR 1910.219(b)(1): Flywheels with parts seven feet or less above floors or platforms were not guarded in accordance with the requirements specified in 29 CFR 1910.219(b)(1)(i) through (b)(1)(iv): (a) Cut, Bend, Form - Department 1 - The flywheel, a maximum of 6 feet above the floor, on the Swaine full-revolution mechanical power press, employer machine number SP1 did not have guarding to cover the entire inner side. In addition, the expanded metal guard on the periphery and right side of the flywheel had openings 1.5 inches high by 1 inch wide; and the expanded metal had gotten removed on the outer side around the shaft, exposing employees to the hazard of getting caught-in rotating parts, on or about October 27, 2022. (b) Cut, Bend, Form - Department 1 - The flywheel, with a bottom height of 51.5 inches above the floor, on the Rousselle Mechanical Stamping Press, model 1, serial number S-829, employer machine number SP2, did not have guarding on the inner side, exposing employees to the hazard of getting caught-in rotating parts, on or about October 27, 2022. (c) Cut, Bend, Form - Department 1 - The flywheel, with a bottom height of 51 inches above the floor, on the Rousselle Stamping Press, model 1A, serial number 700, employer machine number SP7, full-revolution mechanical power press, did not have guarding on the inner side, exposing employees to the hazard of getting caught-in rotating parts, on or about October 27, 2022. (d) Cut, Bend, Form - Department 1 - The flywheel, with the lower half less than 7 feet above the floor, on the L&J Press Corp., employer machine number P5, mechanical power press, did not have guarding on the lower half of the inner side, exposing employees to the hazard of getting caught-in rotating parts, on or about October 28, 2022. (e) Cut, Bend, Form - Department 1 - The flywheel, with the lower half less than 7 feet above the floor, on the part-revolution mechanical power press, employer machine number P2, did not have complete guarding on the lower half of the inner side, exposing employees to the hazard of getting caught-in rotating parts, on or about October 28, 2022. (f) Cut, Bend, Form - Department 1 - The flywheel, with the lower half less than 7 feet above the floor, on the Rousselle, model 10-B-48, serial number HB8A, employer machine number P1, mechanical press brake, did not have guarding on the lower half of the inner side, and did not have guarding on a circular portion on the outer side. The outer side had a set of wooden steps, which allowed access to the circular opening, exposing employees to the hazard of getting caught-in rotating parts, on or about October 28, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
