Text For Citation: 01 Item/Group: 007A Hazard:
29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about March 1, 2022, with respect to an establishment located at 1101 Perry St, Algoma, WI 54201, employees were exposed to hazards associated with moving machine parts when: a) the key controlling the door interlock for the Daewoo Puma 18 CNC Lathe, Serial #PM180010 was left in the machine and allowed an employee to turn off the interlock function. b) employees were using the Bayleigh Type HSP-176M Hydraulic Shop Press, Serial #12 P0033 in the Welding Bay. The Press was operated by a single lever that allowed access to the point of operation while the ram was in motion. c) an opening in the on the frontside of the Scotchman Ironworker, Model D070-24M, Serial #1280M0605 in the Fabrication area allowed employees to access the point of operation while the machine was in motion. d) employees using the AccurPress 130 Ton Hydraulic CNC Press Brake, Model 713010, Serial #10669 were not protected from the moving dies of the presses by any guarding. An employee was observed with their hands on the dies as the machine performed a bend. e) employees using the AccurPress 175 Ton Hydraulic Press Brake, Model 7017512, Serial #8486 were not protected from the moving dies of the presses by any guarding. f) employees using the AccurShear Shear, Model 625010, Serial #5033 were able to access the hold downs of the shear due to the fixed barrier guards having openings that were wide enough to allow access to the points of operation.
