29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: At 8502 Mid County Industrial Drive, St. Louis, Missouri, specifically inside of the Liquid Line Department, the Liquid Line #2 Machine was not completely isolated from all energy sources prior to an employee bypassing the guard to make adjustments to the moving machine parts. This employee received severe lacerations to four fingers on his left hand on or about 07/30/19. This violation was noted on 08/02/19.
