29 CFR 1910.146(d)(8): Designate the persons who are to have active roles (as, for example, authorized entrants, attendants, entry supervisors, or persons who test or monitor the atmosphere in a permit space) in entry operations, identify the duties of each such employee, and provide each such employee with the training required by paragraph (g) of this section; a. On or about May 25, 2018, an employee cleaning the inside of a semi-truck tanker was pronounced dead following a confined space rescue. The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. Where the hazardous atmosphere had not been eliminated prior to entry, a permit required confined space entry was not performed. Employees had not been designated and trained to perform the tasks of confined space entrant, attendant, and entry supervisor in that the measures required to complete a permit-required confined space permit were not performed, the permit was not signed by an entry supervisor and an attendant was not positioned to communicate with the entrant and to monitor the entrant's activities and alert the entrant of the need to evacuate the space or to summon rescue services.
