Buckeye Ready-Mix, LLC located in Marion, Ohio: On or about February 28, 2018, the employer did not ensure the employee acting as the confined space supervisor who signed off on the confined space entry permit for the torching and welding work inside of aggregate bins #2, #3, #4, and #5, had received any permit required confined space training. b. Buckeye Ready-Mix, LLC located in Marion, Ohio: On or about February 28, 2018, the employer did not ensure the employee who was acting as an attendant while employees entered aggregate bins #2, #3, #4, and #5 to perform welding and torching work, had received any permit required confined space training.
