29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (a) Options Medical Center dba Options, 1700 S. 120th St., Lafayette, CO 80026: On or before January 25, 2018, the employer did not have a written Hazard Communication Program that could be provided to OSHA upon request. Chemicals used by employees include, but are not limited to Pure Bright Germicidal Ultra Bleach, isopropyl alcohol, pH Up, pH Down, House and Garden B Aqua Flakes Nutrient, and House and Garden B - Coco Nutrient. This condition exposed employees to chemical hazards.
