This violation was most recently observed on or about December 6, 2017 in the Benzoyl Peroxide (BPO) Unit at United Initiators located in Helena, Arkansas when the employer did not establish and implement written procedures to maintain the on-going mechanical integrity of process equipment; a) Reactor Vent Scrubber S-1101. b)70% Hydrogen Peroxide Day Tank T-1112. c) Mother Liquor Heat Exchanger #1 d) Benzoyl Chloride Bulk Storage Tank T-1103. e) Pressure Safety Relief Valves #408, #410 and #413. f) 12" BzCl-XX-0701, 2" BzCl-XX-0404, and 1/2" H2O2-XX-0405 process piping.
