29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling: 101 North Main Street, Newark, Illinois, west building shop, southwest wall welding area, electrical box (i.e. pull and junction box): on or about May 4, 2017, employees connecting and using connected appliances were exposed to fire and electric shock hazards. Appliances were connected to an electrical box that was not installed in accordance with the listing instructions. The electrical box was not affixed (e.g. wall mounted) and was powered by two other flexible cords connecting in series to each other from a branch circuit receptacle. There is no abatement certification or documentation required for this item.
