29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) BK Tires, Inc. at 3775 Puritan Way, Frederick, CO 80516: On or before July 16, 2015, the employer provided two pendants, each with a metal 4-plex receptacle box, that were not designed for such use, near the copper tubing stand. These receptacle boxes included knockouts that had the potential to expose energized conductors if opened. Condition exposed employees to 120 volt electrical shock hazards.
