29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: ESTABLISHMENT: The screwdriver, which had a metal shaft welded on to extend the length and used by an employee to manually scrape dried glue with a hammer from the rollers of the No. 1 Paper Converting Machine was not in safe condition. On or about 08/27/2015, the above hammer slipped and struck an employee's hand while performing the above task, and the employee sustained laceration and dislocation of a finger.
