29 CFR 1910.146(k)(2)(iv): 29 CFR 1910.146(k)(2)(iv): An employer whose employees have been designated to provide permit space rescue and emergency services did not ensure that affected employees practiced making permit space rescues at least once every 12 months: a) Production areas - Emergency Response Team (ERT) members who may be required to conduct rescues from permit required confined spaces including, but not limited to reactors, tanks, effluent tanks, and blenders did not conduct practice rescues at least annually. On or about 7/21/14. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
