29 CFR 1910.119(l)(5): A change covered by this paragraph resulted in a change in the operating procedures or practices required by paragraph (f) of this section, and such procedures or practices were not updated accordingly: The employer did not update current standard operating procedures and practices related to the installation and intended use of the emergency shutdown devices located outside of the two engine rooms. All of the current emergency shutdown procedures within the standard operating procedures such as but not limited to the Frick #1 Eng. Rm. #1 did not reflect any discussion about the emergency shutdown devices. Employees were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
