29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. Jobsite - On or about September 17, 2012 employees working from a welded frame scaffold had not been trained on how to recognize and prevent scaffold hazards, such as: 1. Planking not overlapped over supports at least 12 inches or restrained to prevent movement 2. Scaffold not fully planked 3. Climbing scaffold bracing to access/egress scaffold
