While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing. (a) Kill Plant - On or about September 18, 2012 open hole used to dispose of dead chickens did not have guarding or a cover exposing employees to strains, broken bones, or fall hazard. (b) Kill Plant - On or about September 18, 2012 open drain in picking room did not have guarding or a cover exposing employees to strains, broken bones, or fall hazard. (c) Kill Plant - On or about September 18, 2012 meat drain in the hallway did not have standard guarding exposing employees to fall hazard.
