This violation occurred on or about April 28, 2012, on the interior west wall of a residential home where employees were exposed to a fall hazard of 12 feet, 4 inches when spraying foam insulation in the living room from the platform of the second/top tier of a mobile scaffold where the pins to hold the cross braces were not working.
