29 CFR 1910.119(j)(4)(i): Inspections and tests were not performed on process eq uipment to maintain its mechanical integrity: A.The employer does not ensure tha t pressure vessels are inspected. This violation was observed on or about 05/24/ 10. As a result of not inspecting the heat exchangers, nuts that are missing on anchor bolts are not discovered and replaced, exposing employees to a hazard of being struck by falling equipment or by flying debris. Some nuts are missing and others are clearly loose. These conditions are located on Heat Exch angers in the Platformer area identified as 18-EX-02B, 18-EX-01B, 19-EX-03B, and 19-EX-06B. One method of abatement among others is to add anchor bolts to exist ing inspection schedules, and then correct the deficiencies found in these inspe ctions. B.The employer does not inspect steel supports at the facility. This vio lation was observed on or about April 2010 when the CHSO observed the fireproofi ng had been removed from vessel supports in the Platformer area to add and remov e I-beams. The fireproofing was not repaired after the modifications. In the eve nt of a fire, the degradation of concrete to the brittle state, can lead to coll apse of the structure. Additionally, rain water gets between the insulation and the I-beams causing excessive corrosion which can lead to collapse. Any collapse could cause struck by hazards to employees in the vicinity. Employees are exposed to fire and explosion hazard s if pipes break and release flammable liquids or gases which are ignited. One m ethod of abatement among others is to add fireproofing to existing inspection sc hedules, and then correct the deficiencies found in these inspections. C.The emp loyer does not inspect grounding cables for deficiencies. This violation was obs erved on or about May 26, 2010, in the Platformer Unit where grounding cables ha d been removed from equipment and not replaced. The grounding strips on Heat Exc hanger 19EX03B, 19-EX-14A, 19-EX-04B and pump 31502 motors Siemens (460 volt mot or order numbers 2144282002-2 and 2144282002-1) in the Platformer area are not c onnected. This condition can expose employees to fire and explosion hazards. One method of abatement among others is to add grounding cables to existing inspection schedules, and then correct the deficiencies found in these inspectio ns. Pursuant to 29 C. F. R. 1903.19, within ten (10) calendar days of the date o f this citation, the employer must submit documentation that it is in compliance with the standard, including describing the steps it is taking to ensure employ ees are not exposed to this struck-by and/or flying debris hazard.
