29 CFR 1910.119(e)(1): The employer did not perform an initial process hazard an alysis (hazard evaluation) on processes covered by 29 CFR 1910.119: This violati on was observed on or about 1/19/2010, where the employer did not ensure that an initial Process Hazard Analysis (PHA) was conducted and completed for the oil & gas separation processes located on the EC2 platform. This condition exposed e mployees to fire, explosion and highly hazardous chemicals (HHC). Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of the citation, the employ er must submit documentation showing that it is in compliance with the standard, including describing the steps it is taking to ensure that a Process Hazard Ana lysis is conducted and documented in accordance with this standard.
