29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process technology and equipment that affect a covered process: a) E2 and S2 Reactor Units - Management of change procedures were not used for changes made by McFoy Refrigeration Inc. such as, but not limited to, programming the control system software as it relates to HVAC ventilation in the E2 Control Room and the installation of a manual emergency damper switch for the S2 Control Room.
