29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment. On or about 25 June 2025, employees of Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, were exposed to mechanical, atmospheric, and thermal hazards while working on or inside an industrial oven. Each authorized employee did not affix a personal lockout device to each energy isolating device of Sugar Coat 1 Oven, to include but not limited to the main electrical panel, picker drive, and conveyor drive.
