29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about November 13th, 2024, and at times prior thereto, employee(s) used a yellow extension cord that was plugged into a 4x4 receptable outlet box leading to a Yellow Jacket Grounded 5 outlets Adapter. The yellow extension cord and the portable floor fan was not visually inspected for exposed wiring. Also, the manufacturers power cord to the Robot Coupe MP450 Turbo 18" Single Speed Immersion Blender was missing a grounding pin. This condition potentially exposed employee(s) to electrical shock and/or fire hazards. In the following location(s): a) Building C - Yellow extension cord b) Champion Kitchen - Robot Coupe MP450 Turbo 18" Single Speed Immersion Blender c) Cart Shop - Portable Floor Fan
