29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy might be supplied to the particular lines and equipment were de-energized / inoperable (i.e. open) and then did not ensure that such means were tagged to indicate that employees were at work: (a) On or about 6/19/2024 PGP gas distribution field Coaling, AL 35449, at Utility Pole # 2080, the employer exposed employees to electrocution hazards while making repairs on broken cutout switch where the employer did not ensure that all equipment was effectively de-energized. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
