29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating. On or about April 24, 2024, employees who operate the MIG welder at the T&T Quality Buildings facility located at 89384 558th Ave, Fordyce, Nebraska were exposed to electrical shock hazards in that the in house fabricated electrical cord powering the Miller Matic MIG Welder was inadequately constructed resulting in due to deteriorated damaged wire insulation at the cord plug.
