29 CFR 1910.146(d)(8): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not designate the persons who were to have active roles in entry operations, identify the duties of each such employee, and did not provide each such employee with the training required: On or about April 26, 2024: Employee(s) engaged in grain handling activities at or about 101 North Franklin Street, Brunswick, Nebraska were exposed to atmospheric hazards in that the employer had employee(s) enter area beneath the Ground Pile flat storage areas; employee(s) were not previously designated and lack the required skills to serve as part of a permit-required confined space entry team.
