29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At a facility located at 127 Claxton avenue, Carthage, MO 64836, employees were exposed to hazards that include caught in, crushed by and amputation. Instances of this exposure include, but are not limited to: Instance 1: The employer exposed the employees to these hazards by allowing them to work on the elevated walking area, near the upper and lower infeeds of the PRO-PAL 3500 AST palletizer machine, while electrically and pneumatically powered components of the palletizer were moving. These components include, but are not limited to, powered rollers, void arms, and row travel bars. Instance 2: The employer exposed the employees to these hazards by allowing them to work near rotating conveyor belts on the elevated walking area, near the upper and lower infeeds of the PRO-PAL 3500 AST palletizer machine. Instance 3: At a facility located at 127 Claxton avenue, Carthage, MO 64836, employees were exposed to hazards that included caught in, amputation and crushed by. The employer exposed the employees to these hazards by allowing them to work near an exposed sprocket wheel and chain that powered the lower conveyor, near the lower infeed, directly adjacent to the elevated catwalk of the AST palletizer.
