29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): At a worksite located at 12149 Hartville Road, Licking, MO: The employer failed to protect employees from struck-by and crushing. This was most recently demonstrated on or about December 7, 2023, and at times prior to when employees were allowed to operate powered industrial trucks and had not been deemed competent to operate the equipment by their employer through successful completion of a training program.
