29 CFR 1910.147(c)(4)(i): Energy control procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about 05/05/2023, at the Shell Chemical facility located in Deer Park Texas, employees were exposed to fire and chemicals release hazards when the employer did not ensure energy control procedures were utilized for the control of potentially hazardous energy for the following but not limited to: a.) The employer did not verify that the breaker of MOV-2514 (motorized operated valve) was locked per the electrical energy isolation procedure (S-104-Procedure for Locking Out Electricity Driven or Controlled Equipment). b.) The employer did not ensure that two lead craft foremen (from Turner Industries Group and Universal Plant Services) attached their locks to the multiple padlock hasp of the MOV-2514 breaker per the electrical energy isolation procedure (S-104-Procedure for Locking Out Electricity Driven or Controlled Equipment).
