29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Mechanical Room; On November 18, 2022, and times prior to, the employer did not ensure employee(s) performing service and maintenance of the Trane and York Chillers in the mechanical room were provide equipment specific procedures to identify and isolate energy sources, exposing employees to struck-by and/or electrical hazards. (b) Mechanical room; On November 18, 2022, and times prior to, employees performing maintenance on a boiler system were not provided equipment specific procedures to lock out sources of hazardous energy hazards, exposing employees to struck-by and/or electrical hazards.
