29 CFR 1910.119(j)(4)(i): The employer did not perform inspection and tests on process equipment. On or about 11/10/2022, at the Schirm facility located in Ennis Texas, employees were exposed to chemical release hazards when the employer did not perform inspection and testing on process equipment for the isopropylamine systems including but not limited to: a. Two pressure relief valves (PRVs) for the isopropylamine (IPA) tank b. One PRV-01 for reactor R-1 c. One PRV-02 for reactor R-2 d. One PRV on a discharge pipe of the IPA pump P-02 e. One PRV on discharge pipe of the IPA pump P-03 f. Isopropylamine pipelines from IPA tank to pumps P-02 & P-03 g. Isopropylamine pipelines from pumps P-02&03 to reactors R-1& R-2
