29 CFR 1910.303(c)(3)(i): All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose. a) Wheelhouse - On or about July 20, 2022, and times prior thereto, conductors of TSGU-4 10AWG/4 power cable were spliced to conductors of a BC-5W2 marine boat cable with use of a splice connector and were not covered with an insulation, exposing employees to electrical and fire hazards. b) Wheelhouse - On or about July 20, 2022, and times prior thereto, conductors of a BC-5W2 14AWG/2 marine boat cable were spliced with conductors of a blue strobe light and a bell alarm manufactured by Aqualarm for hardwiring and were not covered with insulation, exposing employees to electrical and fire hazards. c) Stateroom 4 - On or about July 20, 2022, and times prior thereto, electrical tape was used to cover electrical conductors for flexible wiring of an exhaust fan and heater spliced to a BC-522 14AWG/2 flexible cord exiting a hole in the ceiling providing power from a circuit breaker, exposing employees to electrical and fire hazards. d) Galley Kitchen - On or about September 29, 2022, and times prior thereto, a braided flexible cord on top of the oven exhaust was observed spliced at ends with wire gauge caps installed and electrical tape placed over parts of the conductors, exposing employees to electrical and fire hazards. e) Pump room - On or about September 29, 2022. and times prior thereto, flexible cords were observed spliced for a quadplex receptacle unit adjacent to the exhaust stack. Electrical tape was placed over conductors, exposing employees to electrical and fire hazards. f) Stateroom 2 - On or about October 25. 2022, and times prior thereto, the free ends of conductors of an overhead light receptable located above the sink were covered with electrical tape, exposing employees to electrical and fire hazards. g) Stateroom 2 - On or about October 25, 2022, and times prior thereto, a 10AWG braided sheathed cable had free ends of three conductors exposed and covered with electrical tape, exposing employees to electrical and fire hazards. h) Stateroom 2 - On or about October 25, 2022, and times prior thereto, electrical conductors of a 14AWG flexible cord were spliced to electrical conductors in an open junction box receded into the wall near the ceiling and were not covered with insulation, exposing employees to electrical and fire hazards. i) Stateroom 2 - On or about October 25, 2022, and times prior thereto, a light fixture at the ceiling was hardwired with the conductors of the fixture and the conductors of a light switch below spliced and connected to 14 AWG flexible cord secured to the ceiling area and was not covered with insulation, exposing employees to electrical and fire hazards. j) Stateroom 2 - On or about October 25, 2022, and times prior thereto, a 14 AWG flexible cord painted white was observed attached to the ceiling and had conductors spliced without proper insulation covering the open ends of the conductors, exposing employees to overhead electrical hazards. k) Stateroom 8 Storage - On or about October 25, 2022, and times prior thereto, a 14 AWG flexible cord was spliced and connected to a light fixture and light switch receptable at the ceiling area and the conductor ends were covered with electrical tape, exposing employees to electrical and fire hazards. l) Stateroom 9 - On or about October 25, 2022, and times prior thereto, a 14 AWG flexible cord was observed spliced with the conductors covered with painters' tape, exposing employees to electrical and fire hazards.
