29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 3, 2021, employees were exposed to struck-by hazard associated with operating continuous casting machine (CCM) #1 and its associated turn table to fill cone molds with polymer concrete in that: (a) The turn table was not guarded on its right side; (b) The turn table was not guarded on its left side.
