29 CFR 1910.119(j)(4)(iv):The employer did not document the date of inspection, the name of the person performing the inspection, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection, and the results of the inspection for each piece of process equipment. a. Mechanical Integrity - On or about 14 December 2021, the employer failed to document that 5 year Mechanical integrity inspection and test on process equipment were documented in accordance with IIAR 6 section 5.1 to show the name of the inspector and a description of the inspection/test being conducted, failure to accurately detail the inspections are conducted by qualified persons in accordance with relevant RAGAGEP, can result in equipment failure, ultimately resulting in release of ammonia that can cause death, fire, asphyxiation or explosion. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
