29 CFR 1910.178(l)(4)(ii)(E): Refresher training, including an evaluation of the effectiveness of that training shall be conducted when a condition in the workplace changes in a manner that could affect safe operation of the truck: a. 1 Silverline Dr., North Brunswick, NJ - Assembly/Warehousing area The employer did not provide refresher training to employees operating powered industrial equipment where changes to the workplace affected safe operation of the truck. Employees and staffing agency employees engaged in the manufacture and warehousing of vinyl window products operated equipment that included, but was not limited to, Komatsu FG25ST forklifts and Jungheinrich Model ERC327 electric pallet jacks, where the placing of products on carts in the operating area created a hazard where employees were injured when operating powered industrial equipment that required turning in narrowed aisles and were not provided training for safe operations in the highly congested areas.
