A) FAZTEC Industries - 200 Bloomfield Ave, Staten Island NY 10314 The employer did not conduct proper inspection and repair of conductors. Energized conductors operating at 460VAC were damaged exposing energized 2 phases of the copper conductors. Condition noted on or about August 27, 2021
