29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about April 14, 2021, at the batch plant, employees working from an Ingersoll-Rand rough terrain forklift (Model: VR-642C, Serial: 189738) were exposed to fall and struck-by hazards in that the employer used a wire mesh pallet basket to lift and or elevate workers.
