29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment: a. On or about October 15, 2020, and at times prior, in the Press Department, the employer had not established and followed a program of periodic and regular inspections on the Stokes R4 Part-Revolution Mechanical Power Press to ensure the press was in safe operating condition. It was found that safeguards were not maintained in place, and parts and equipment were not ensured in safe operating condition, thereby exposing employees to struck-by and caught-in/between hazards during operation and die setting.
