29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Throughout the facility: On or about February 7, 2020, employees used Yale sit down forklift Model GLC155, SN: A879D01606W with a company made platform to lift employees to perform maintenance tasks. Employees were exposed to falling from distances up to 4-feet to the concrete below. The manufacturer did not approve the use of the platform.
