29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about 1/13/2020, in the production area at 2555 Moreland Avenue, Atlanta, GA: Employees elevated in a work platform attached to the forks of a powered industrial truck to perform maintenance on the facility or equipment are exposed to a fall hazard at heights of approximately 10 feet or more from ground level. The employer failed to obtain written approval from the manufacturer of a powered industrial truck for modifications and/or additions if the modifications and/or additions affect the capacity and safe operation of the truck.
