29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: a. On or about 12/12/2019, at the fabrication area near the MEP bending machines, employees were exposed to electrical hazards, in that, a 240 volts main power disconnect was missing a panel cover and had 3 breaker openings without a cover. b. On or about 12/12/2019, at the fabrication area near column N-11, employees were exposed to electrical hazards, in that, a 240 volts control panel had an opening of 3 inches by 2 inches without a cover. c. On or about 12/12/2019, at the fabrication area near MEP 91, employees were exposed to electrical hazards, in that, a 240 volt red breaker box had a knockout missing from the side of the box.
