29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: (a) PRODUCTION AREA - Employees were required to perform lock out tag out on different pieces of equipment in the facility including the six aspirators, multiple conveyors, and various sorting facility equipment. The employer provided general LOTO training but failed to fully document machine specific LOTO training for employees on the specific methods or the locations of the electrical disconnects that were to be used to de-energize the equipment, on or about August 15, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
