29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a) On or about July 23, 2019, and times prior thereto, the employer failed to ensure the piping was labeled and flow directions were indicated on the piping system IAW American Society of Mechanical Engineer (ASME) A13.1, 2007, Scheme For The Identification of Piping Systems. Employee(s) were exposed to the hazards associated with propane and butane such as burns, fire, explosion and asphyxiation. b) On or about July 23, 2019, and times prior thereto, the employer failed to ensure the piping system was protected from being struck by equipment. The piping system near the gas house was struck by moving equipment such as, but not limited to, a fork lift and/or aerial/scissor lift. Employee(s) were exposed to the hazards associated with propane and butane such as burns, fire, explosion and asphyxiation. c) On or about July 23, 2019, and times prior thereto, the employer failed to ensure the MSA Ultima gas monitors were calibrated per the manufacturer and Per QuestSpecialty own standard operating procedures. Employee(s) were exposed to the hazards associated with propane and butane such as burns, fire, explosion and asphyxiation.
