29 CFR 1910.178(m)(5)(ii): A powered industrial truck is unattended when the operator is 25 ft. or more away from the vehicle which remains in his view, or whenever the operator leaves the vehicle and it is not in his view. a) Warehouse, row between M16 and M17 - On or about April 25, 2019, an employee left a Raymon battery Stand up, Model number 960-CSR30T, SN # 960-12-02047 unattended while going to the bathroom and did not have the forklift in his view. b) Warehouse repair workshop - On or about April 25, 2019, an employee left a Raymon battery Stand up, Model number 960-CSR30T, SN # 960-13-02715 unattended while the operator turn his back to enter information on the computer and did not have the forklift in his view. c) Dock warehouse - On or about April 25, 2019, an employee left the Raymon battery sit-down model 445C30QM, SN # 445-10-10192 unattended while the operator turn her back to pick up boxes and did not have the forklift in her view. d) Warehouse - On or about April 25, 2019, an employee left the Raymon battery stand up model 560-0P030TT, SN # 560-12-A14817 unattended while the operator left for lunch. e) Warehouse - On or about April 25, 2019, an employee left the Raymon battery stand up model 560-0P030TT, unattended while the operator left for lunch. f) Warehouse - On or about April 25, 2019, an employee left the Raymon battery sit-down model 445C30QM, SN # 445-12-10900 unattended while the operator turn her back to pick up boxes and did not have the forklift in her view g) Warehouse - On or about April 25, 2019, an employee left the Raymon battery stand up model 560-0P030TT, SN # 560-10-A10412 unattended while the operator went to gather materials. h) Battery charge station- On or about April 25, 2019, an employee left the Raymon battery stand up model 960-CSR30T, SN # 960-12-02045 unattended while the operator left the forklift for it to be charged.
