29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: (a) Site: In Building 1, On the Coating Line, At the aisle side of the Main Drive component, in the facility at 130 Riverdale Street, Northbridge, MA 01534: On 4/3/2019 the guard intended to prevent access to the point of operation on the Main Drive component was a hinged gate/door and could be easily circumvented due to the lack of a locking mechanism or other effective means to prevent unauthorized access.
