29 CFR 1910.261(b)(1): The employer did not ensure that the main power disconnect switch or valve, or both, controlling its source of power or flow of material, was locked out or blocked off with padlock, blank flange, or similar device before conducting any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that required entrance into or close contact with the machinery or equipment: (a) Converting - On or about August 24, 2018 maintenance employee(s) were allowed to trouble shoot machine operation problems from the inside of converting machines, such as the Amica Matrix, while the machine was operating. (b) Converting - On or about August 24, 2018 employee(s) were allowed to clean machines on line 5, 6, and 7 without locking out energy sources. (c) Converting - On or about August 24, 2018 employee(s) were allowed to clear jams on lines 5, 6, and 7 without locking out energy sources. (d) Converting - On or about August 24, 2018 employee(s) were allowed to remove glue build-ups from log saw without locking out energy sources. (e) Converting - On or about August 24, 2018 employees(s) thread line 5 without locking out the machine. The above conditions exposed employees to amputation hazards.
