29 CFR 1910.217(c)(3)(viii)(c): Each two handed trip did not protect the operator, in that the safety distance between each two hand control device and the point of operation was not greater than the distance determined by the formula. 125 Gloucester Ave, Gloucester, MA; Press shop, building 1, and glass shop in basement, building 1 Employees operating power presses with two hand trips were exposed to amputation hazards from two hand trips that were located within the minimum safety distance, including but not limted to the following instances; c) On or about September 6, 2018, the Moly Hole press, shop ID 24103, had two hand trips that were within the calculated distance of 17.2 inches. The buttons were approximately 13.25 inches from the point of operation. d) On or about August 1, 2018, the Moly press, shop ID 24104, had two hand trips that were within the calculated distance of 40.2 inches. The buttons were approximately 15.5 and 15 inches from the point of operation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
