29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) Pallet Mill; On or about November 14, 2017 and at times prior thereto the employer exposed employees to electrical shock hazards in that employees were allowed to use a microwave that was plugged into a 110 VAC outlet that did not have a ground.
