29 CFR 1910.120(q)(2)(ix): The emergency response plan did not address, to the extend not addressed elsewhere, emergency alerting and response procedures: (a) On or about September 1, 2017 employees were exposed to inhalation, fire, and explosion hazards associated with a release of anhydrous ammonia when the employers Emergency Response Plan did not provide procedures for alerting all affected employees, including employees working in the main building, guard gates, and other buildings on the campus about emergencies; (b) On or about September 1, 2017 employees were exposed to inhalation, fire, and explosion hazards associated with a release of anhydrous ammonia when the employer did not alert all affected employees of the location of an ammonia leak.
