29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): On or about 7/17/2017 at the out-feed rollers of the truss fabrication table, operators were exposed to caught-by and amputation hazards at the un-guarded 1/2-inch belts and 4-inch pulleys, 33-inches above the walking/working surface while moving the trusses onto the rollers or walking at or around the rollers at: a. the 3rd driven roller from tail b. 6th driven roller from tail c. 7th driven roller from tail d. 8th driven roller from tail e. 11th driven roller from tail f. 12th driven roller from tail
