29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about 1 May 2017 and at times prior thereto, employees performing box making operations were not protected by machine guarding methods from nip points and rotating parts, exposing the employees to the hazard of being struck by or caught in moving machine parts. A) On the Jumbo Slotter/Printer- nip points at the layboy conveyor, stacker conveyor, register motor. Rotating knives on the slotter heads. Pinch points at the stacker conveyor base pivot point. B) On the Isowa Slotter- nip points at the stacker conveyor and pinch points at the stacker base pivot point and railing. C) On the Large Gluer- rotating catch points from the rollers at the feed end of the machine. Nip point between operator's platform and loader table.
